Who needs Form 1A to renew a licence in Thiruvananthapuram
A private car or two wheeler licence needs Form 1A at renewal only after you have turned 40. A transport licence needs it at every renewal, whatever your age.
Form 1A applies to you in these cases:
- you are 40 or older and renewing a non transport licence
- you want a transport licence, its renewal, or a transport class such as taxi, bus or goods vehicle added, at any age
- you are applying for a learner's licence for a transport vehicle
- you are 40 or older and applying for a new driving licence
- you are applying for an International Driving Permit (IDP)
Under 40 with a private licence, you do not need Form 1A to renew, unless you are applying for an adapted vehicle licence due to a physical disability. If your Kerala application asks for any other medical document, follow what it asks or check with your RTO.
Our Form 1A service covers driving licence renewals and International Driving Permit applications, but not new or learner's licences.
Licence validity after you renew
For a private licence, your age on the day of renewal sets how long the new licence runs, under Section 14 of the Motor Vehicles Act as amended in 2019. Transport and hazardous goods licences run for fixed periods.
| If you renew | The new licence runs |
|---|---|
| A private licence before age 30 | Until age 40 |
| A private licence at 30 to under 50 | 10 years |
| A private licence at 50 to under 55 | Until age 60 |
| A private licence at 55 or older | 5 years |
| A transport licence | 5 years |
| A licence for hazardous goods | 3 years |

Things to check before paying
The ₹1,349 fee cannot be refunded, so settle these points before you pay.
"What about Kerala's extra eye standards?"

"What if the doctor does not issue it?"
Issuing Form 1A is the doctor's decision. Payment of ₹1,349 for the process is non refundable, so there are no refunds, including when the certificate is not issued. Please read our refund and cancellation policy before you order.
"My RTO wants a certificate from a particular doctor."
Then do what your RTO says. Acceptance depends on local RTO rules, so ask before you book.
"Could the doctor fix a wrong address or photo on my form?"
No. The doctor does not change the address, photo or any other detail on Form 1A, so check them before you download it.
"Can I get a printed copy for the counter?"
No. You get only the digital copy, which works when uploaded online.
"I need a new licence, not a renewal."
Our service does not cover new or learner's licences.
Renewing a Kerala licence on Sarathi
Kerala licence renewal is applied for online on Sarathi. The Motor Vehicles Department moved renewal online from 1 January 2021, with scanned copies of the certificates needed uploaded with the application, and since 1 April 2021 Form 1A itself has been issued electronically on the portal.
On Sarathi, select Kerala as your state.
Choose Apply for DL Renewal on the Kerala Sarathi page and follow the prompts for your licence details.
Complete the application for renewal.
Note the application number, then download your Form 1A from your profile with the OTP.
Upload the signed Form 1A and any other documents your application lists, and pay the fee it shows.
Track the application status on Sarathi.
The Apply for DL Renewal label is as it appeared on the Kerala Sarathi page in October 2026, and labels can change. If your licence details are not on Sarathi, ask your RTO what to bring. If your licence expired more than a year ago, check the time limits below.
RTO and sub RTO offices for Thiruvananthapuram
Kerala's Motor Vehicles Department lists a Regional Transport Office for the city, RTO Thiruvananthapuram (KL 01), though renewal itself is applied for online on Sarathi. Since 2019 the Motor Vehicles Act lets you apply for a driving licence to any licensing authority in the state.
The Motor Vehicles Department's office directory includes the offices below in Thiruvananthapuram district. The directory does not say which areas each office serves, so confirm which one covers your address before you visit. Offices and the areas they serve can change.
| Office | Code |
|---|---|
| Regional Transport Office Thiruvananthapuram | KL 01 |
| Regional Transport Office Attingal | KL 16 |
| Sub Regional Transport Office Parassala | KL 19 |
| Sub Regional Transport Office Neyyattinkara | KL 20 |
| Sub Regional Transport Office Nedumangad | KL 21 |
| Sub Regional Transport Office Kazhakootam | KL 22 |
The codes are the registration codes the directory gives for each office.

Mistakes to avoid with Form 1A
A Form 1A is valid for six months from issue, and a printout of the digital copy is not accepted offline. Check this list before you upload.
Older than six months.
The certificate lapses six months after issue, so a renewal that has stalled needs a new one.
Not the generated form.
Form 1A has to be issued electronically on the portal, so use the Form 1A generated for your Kerala application.
Wrong details on the form.
The doctor does not change the address, photo or any other detail on Form 1A, so check them before you download it.
A wrong application number.
Copy it from your Sarathi application.
A printout at the counter.
Upload the digital copy online, since a printout of it is not accepted offline.
Your RTO's own rules.
Acceptance depends on local RTO rules, so ask your RTO if you are unsure.
Hazardous goods licence.
Form 1A also needs reaction time, side vision and glare recovery checks, so see a doctor in person.
The official Form 1A PDF and the Kerala form
The official blank Form 1A is published on Parivahan. Download the official Form 1A PDF to see what the form asks.
That national PDF has no squint or fundus questions and checks side vision only for hazardous goods, so it falls short of Kerala's extra standards. For your renewal, a doctor attests the Form 1A generated for your Kerala application, because the certificate has to be issued electronically on the portal.
What the national form asks
| On the form | What it asks or certifies |
|---|---|
| Applicant details | Your name, whose son, wife or daughter you are, permanent address, date of birth and two identification marks, with a photo across which the doctor signs, so that part of the signature falls on the photo and part on the certificate |
| 3(a) | Whether there is a defect of vision, and if glasses correct it |
| 3(c) | Reading a motor car number plate at 25 metres in good daylight. The form has no 3(b) |
| 3(d) | Whether deafness stops you hearing ordinary sound signals |
| 3(e) | Night blindness |
| 3(f) | A defect, deformity or loss of a limb that affects driving |
| Optional | Blood group and Rh factor |
| Doctor's certificate | The certificate wording refers to a personal examination, with attention to distant vision, hearing, and arms, legs, hands and joints |
| Colour vision | Checked on a standard Ishihara chart, with no severe or total colour blindness |
| Hazardous goods only | Reaction time, side vision and glare recovery |
| Conclusion | The doctor's finding of fit or not fit to drive a vehicle other than an adapted vehicle, with reasons when not fit. Then the doctor signs and adds name, designation, registration number, seal and date, and you sign or give a thumb impression |
One note on the form says that someone who cannot speak but can hear may still be certified for a non transport licence.

Questions about Form 1A in Trivandrum
Is Form 1A required for driving licence renewal in Thiruvananthapuram?
Yes, when you renew a private licence at 40 or older, and at every renewal of a transport licence. The requirement comes from Section 15 of the Motor Vehicles Act and Rule 5 of the Central Motor Vehicles Rules. Private licence holders under 40 do not need Form 1A to renew, unless applying for an adapted vehicle licence due to a physical disability.
Is there an eye test for driving license renewal in Trivandrum?
Yes, when your renewal needs Form 1A, because the form asks about vision: distant vision, number plate reading at 25 metres, night blindness and colour vision. Kerala's form lists extra visual standards, including squint, field of vision and fundus. With us you take an eye and ear self test online, and a doctor then attests your Form 1A.
What visual standards does Form 1A include in Kerala?
Kerala's Form 1A includes extra visual standards: visual acuity, night blindness, squint, field of vision and fundus. The national form already asks about distant vision, number plate reading at 25 metres, night blindness and colour vision. The doctor may ask for more details, and issuing the certificate is the doctor's decision.
Where can I download the Form 1A PDF for a Kerala application?
For a renewal, download the Form 1A generated for your application from your profile on the RTO website, using your OTP. The blank national PDF on Parivahan helps you read the questions, but it does not show Kerala's extra checks, and the certificate must be issued electronically on the portal.
Is Form 1 still needed for a Kerala driving licence?
No. Under the central rules, Form 1 was omitted from 1 April 2021, and the self declaration of physical fitness now sits in Form 2. Form 1A is a separate document: the medical certificate a doctor attests, needed to renew a private licence from age 40 and for transport licences.
Who can sign Form 1A in Kerala?
The form says it is to be filled in by a registered medical practitioner appointed for the purpose by the State Government, or a person authorised in this behalf by the State Government. If your RTO names particular doctors, go by that. Acceptance depends on local RTO rules, and we cannot promise acceptance.
How long does a Form 1A certificate stay valid?
Six months, counted from the day the doctor issues it, in every state. Notification G.S.R. 240(E) cut the validity under Rule 5(3) from one year to six months, with effect from 1 April 2021. After six months you will need a fresh Form 1A.
Is Form 1A needed to renew a private licence before 40?
No, not under the national rules: Form 1A is needed for a private car or two wheeler licence only from age 40, unless you are applying for an adapted vehicle licence due to a physical disability. Transport licences need Form 1A at any age. If your Kerala application asks for any other medical document, follow what it asks or check with your RTO first.
How do I renew my driving licence in Kerala online?
Apply on Sarathi after selecting Kerala, using the Apply for DL Renewal option. Kerala has taken renewal applications online since January 2021. Fill in the application, save the application number, and download your Form 1A if you need one. Then upload the signed form, pay the fee Sarathi shows and track the application.
What is the driving licence renewal fee in Kerala?
Under Rule 32 of the Central Motor Vehicles Rules, renewal is ₹200 plus ₹200 for the smart card. Applying more than a year after expiry costs ₹300 plus ₹1,000 for each year or part of a year, and a driving test is ₹300 per class. Kerala may add service charges, so pay what Sarathi shows.
What does MedicalCertificate.in charge for Form 1A?
₹1,349 for the self test option, with the signed digital Form 1A sent on WhatsApp or email. You pay before work starts, and payment of ₹1,349 for the process is non refundable, so confirm that your renewal needs Form 1A first. Allow up to 72 working hours* after you share your application number, the downloaded Form 1A PDF and complete the ear and eye self test. We work Monday to Saturday from 10:30 am to 8:00 pm IST. We do not offer printed or physically attested copies.
Which transport office handles licences in Thiruvananthapuram?
Thiruvananthapuram has a Regional Transport Office of Kerala's Motor Vehicles Department, but renewal itself is applied for online on Sarathi. Since 2019 the Motor Vehicles Act lets you apply to any licensing authority in the state. The department's directory lists the offices in and around the city, so confirm which one covers your area before a visit.
