Since June 2024 the Tamil Nadu transport commissioner has required the doctor to be registered with the Tamil Nadu Medical Council and enrolled on Sarathi, so the doctor uploads the certificate directly. Doctors enrol on Sarathi once, using their council registration number and clinic details. A doctor who is not empanelled cannot issue Form 1A in Tamil Nadu.

The official form asks about defects of vision and whether glasses correct them, reading a number plate at 25 metres in daylight, deafness, night blindness, and any loss or deformity of limbs. The doctor also certifies distant vision, hearing, arms, legs and joints, and colour vision on an Ishihara chart.

In Tamil Nadu the doctor approves Form 1A on the portal, so you do not upload it yourself. After you apply for renewal on Sarathi, the doctor attests the Form 1A generated for your application number. This works only if the mobile number on your application is linked to your Aadhaar.

The Tamil Nadu transport department lists ₹500 to renew a non transport licence and ₹450 for a transport licence, without saying what the amount includes. The central Rule 32 fees are ₹200 for renewal and ₹200 for the smart card, with a late fee after one year. Sarathi shows the final amount before payment.

The RTO fee on Sarathi is separate. We do not provide printed or physically attested copies.

The rules require Form 1A to be issued electronically on the portal, and our doctor attests the form Sarathi generates for your application. Acceptance still depends on your RTO. If it calls you for physical verification, you need a physically attested Form 1A, and a printout of the digital copy will not do.
