Form 1A Medical Certificate for Driving Licence Renewal in Noida

Yes, if you are 40 or older and the licence covers a private car or two wheeler, or if it includes any transport vehicle. The requirement comes from Section 15 of the Motor Vehicles Act read with Rule 5 of the Central Motor Vehicles Rules. Below 40, a private renewal does not need Form 1A.

Most of it is not yours to fill. Sarathi generates the Form 1A for your application, and the form is filled in by a registered medical practitioner appointed for the purpose by the state government, or a person authorised in this behalf by it. You supply accurate details and two identification marks. Do not tick the medical answers yourself.

Form 1A covers vision and whether glasses correct it, reading a car number plate at 25 metres in daylight, hearing ordinary sound signals, night blindness, and limb problems that affect driving. Its certificate also refers to colour vision on an Ishihara chart. Hazardous goods licences add reaction time, side vision and glare recovery, so see a doctor in person.

Yes. Uttar Pradesh is listed on Sarathi for online services, so you can apply for renewal there and select Uttar Pradesh as the state. At 40 or above, or with a transport licence, you also upload a signed Form 1A. You may still be called in, for example for a driving test after a long lapse.

Download the Form 1A generated for your renewal application from your profile on the transport portal, using an OTP. The blank national Form 1A is on Parivahan, but use it only to read the fields. The form comes from the Central Motor Vehicles Rules, which apply in Uttar Pradesh as in every other state.

The Uttar Pradesh Transport Department handles driving licences for Noida. We do not list an address or registration code for the Noida office because we could not confirm them from an official source. Yes, as long as you use it within six months of the issue date. Rule 5(3) of the Central Motor Vehicles Rules, as amended from 1 April 2021, limits Form 1A to six months. Before that change it lasted one year. If your renewal slips past the six month mark, get a fresh certificate.

No. Our digital Form 1A works only when uploaded online, and a printout of it is not accepted offline. If your RTO calls you in for physical verification, you will need a Form 1A physically attested by a doctor you visit, which we cannot provide. Upload the digital copy to your Sarathi application instead.

Our self test with a digital Form 1A costs ₹1,349 and is non refundable. The transport fee is separate: Rule 32 sets ₹200 for renewal and ₹200 for the smart card. Renewing more than a year after expiry costs ₹300 plus ₹1,000 for each year or part of a year, and any driving test is ₹300 per class.

A private car or two wheeler licence does not need it before 40. This follows Rule 18(1)(d) of the Central Motor Vehicles Rules, as substituted by G.S.R. 240(E). Transport licences are the exception: Form 1A is needed at every renewal, for a transport learner's licence, and when you add a transport class, whatever your age.
