Who needs Form 1A in Lucknow
Whether you need Form 1A depends on two things: your age when you renew, and whether your licence covers a transport vehicle. Private licence holders under 40 do not need it.

Can you generate Form 1A yourself?
No. Sarathi generates a Form 1A for your renewal application, but the medical part is filled in by a registered medical practitioner appointed for the purpose by the state government, or a person it authorises.
You download that Form 1A from your profile on the transport portal, using an OTP. The practitioner then completes the medical questions and adds a signature, name, designation, registration number, seal and date.
Do not upload a Form 1A that you, or a software tool, filled in, because the form is meant to be completed by the practitioner it names. Sarathi itself warns users to beware of fraudulent websites and apps.
How Uttar Pradesh handles Form 1A
Uttar Pradesh follows the national Form 1A rules, and licence renewals there run through Sarathi. With our service you attach the signed digital Form 1A to your Sarathi application yourself, since Uttar Pradesh is not among the four states where the empanelled doctor approves Form 1A on the portal.

Common doubts before booking
Since the self test fee is non refundable, these are the questions to settle first.
How does the online test work?
You test your own eyes and ears on camera, and a doctor then attests the Form 1A. Issuing it remains the doctor's decision, and the online test cannot stand in for a doctor's visit.
My office wants a government doctor.
Then follow your office. Acceptance depends on local RTO rules, so check before you pay.
Can the doctor fix a wrong date of birth?
No. Details on Form 1A, such as the address and photo, stay as they are because the doctor does not change them, so check yours before ordering.
I need it for a learner's licence.
We cannot help with that, as our Form 1A covers renewals and International Driving Permit applications only.
How a UP licence renewal runs on Sarathi
A Lucknow renewal is made on the Sarathi portal with Uttar Pradesh chosen as the state.
Visit sarathi.
parivahan. gov. in and select Uttar Pradesh, which is on the portal's list of states with online services. Open the renewal service for your driving licence and give the licence details it asks for.
Fill in the application with the details the portal asks for.
Save your application number and download the generated Form 1A with your OTP.
Upload the signed digital Form 1A once a registered doctor has attested it.
Pay the fee on the portal.
If your licence expired more than a year ago, renewal depends on passing the test of competence to drive.
Transport offices for Lucknow
This page does not give a Lucknow office address or registration code, because we could not confirm them from an official Uttar Pradesh source. Start your renewal online on Sarathi instead.
Under Section 9(1) of the Motor Vehicles Act, as amended in 2019, you may apply for a driving licence to any licensing authority in Uttar Pradesh.
If an office asks you to come in, confirm its address directly with the UP Transport Department before you go.
UP licence renewal fees and time limits
The central fee for renewing a licence is ₹200, with another ₹200 for the smart card, under Rule 32. Renewing no later than a year after expiry avoids both the late fee and a driving test.

Time limits under Section 15
Earliest:
one year before the expiry date.
After expiry:
the 30 day period in which an expired licence stayed valid no longer exists, because the 2019 amendment removed it.
Up to one year late:
you can still renew without a driving test.
More than one year late:
the licensing authority shall refuse renewal unless you pass the test of competence to drive.

What Form 1A asks and certifies
The form has five yes or no questions about the eyes, ears and limbs, a certificate that refers to a personal examination, and a final conclusion of fit or not fit.

The printed certificate refers to a personal examination, with attention to distant vision, hearing, and the arms, legs, hands and joints. It also states that colour vision was tested on a standard Ishihara chart, with no severe or total colour blindness. For hazardous goods licences, it adds reaction time, side vision and glare recovery.
Your own part of the form holds your name, whose son, wife or daughter you are, your permanent address, date of birth and two identification marks, next to a photo, with part of the doctor's signature over the photo and part on the certificate. A note on the form says a person who cannot speak but can hear may be given a certificate for a non transport licence.

For the renewal itself, a doctor attests the copy that Sarathi generated for your application, because the rules require Form 1A to be issued electronically on the portal.
