The rule sits in Section 15 of the Motor Vehicles Act and Rule 5 of the Central Motor Vehicles Rules. A private licence holder under 40 does not need it, unless applying for an adapted vehicle licence due to a physical disability.

The printed instruction on Form 1A reads: "To be filled in by a registered medical practitioner appointed for the purpose by the State Government or person authorised in this behalf by the State Government". If your RTO asks for a certificate from a particular doctor, do as it says.

The central fees in Rule 32 are ₹200 for renewal and ₹200 for a smart card licence. Apply more than a year after expiry and it becomes ₹300 plus ₹1,000 for each year or part of a year, and a driving test costs ₹300 per class. Gujarat may show its own total, so check Sarathi.
