Is Form 1A required for you?
It is required when you are at least 40 and renewing a private licence, and for any licence that includes a transport vehicle. Private licence holders under 40 skip it.

Renewing a private licence before 40 does not require Form 1A, unless you are applying for an adapted vehicle licence due to a physical disability. The separate Form 1 self declaration was dropped in 2021, and the declaration now sits in Form 2.

Conductor licence medical form in Gujarat
Form 1A is the medical certificate prescribed for driving licences under Rule 5 of the Central Motor Vehicles Rules. A conductor's licence is a separate licence: Section 30 of the Motor Vehicles Act says its application must carry a medical certificate in the prescribed form, signed by a registered medical practitioner. Sarathi's Gujarat page lists conductor licence services separately, so ask your RTO which medical form it needs. Your age at renewal and the new licence period
Section 14 of the Motor Vehicles Act, amended in 2019, fixes how long a renewed licence runs, based on your age on the renewal date.

Form 1A rules that apply in Gujarat
Form 1A is a form under the Central Motor Vehicles Rules, which apply across India. Its first instruction says it is to be filled in by a registered medical practitioner appointed for the purpose by the State Government or person authorised in this behalf by the State Government.

Under Rule 5(4) of the Central Motor Vehicles Rules, Form 1A shall only be issued electronically on the portal.
You upload it.
For a Gujarat application, you add the signed digital Form 1A to your Sarathi renewal. Some other states, such as Tamil Nadu, Maharashtra and Odisha, allow only doctors empanelled with the transport department to issue Form 1A, and they approve it on the portal.
Acceptance depends on your RTO.
If the office handling your renewal names a doctor or a type of doctor, go by that.

"My Form 1A shows an old address. Can the doctor change it?"
No. The doctor does not change the address, photo or other details. Check them before you order.
"Can I write in the answers myself?"
No. The form says it is to be filled in by a registered medical practitioner appointed for the purpose by the State Government or person authorised in this behalf by the State Government. Your part is downloading the Form 1A generated for your application.
Licence renewal on Sarathi for Gujarat
Gujarat runs licence renewal through Sarathi. Its state page, headed Transport Department, Government of Gujarat, lists renewal under Services on DL (Renewal/
Nationally, the Ministry of Road Transport and Highways issued notification S.O. 4353(E) on 16 September 2022 for providing 58 citizen centric services completely online. They relate to driving licences, conductor licences, vehicle registration and permits, and Aadhaar authentication for them is voluntary.
Choose Gujarat.
On Sarathi, select Gujarat from the state list.
Open the DL services.
Pick renewal from the services on DL and enter what the form asks for.
Confirm your identity.
Use whichever identity option Sarathi offers you at this step.
Keep the application number.
Use Find Application Number under Others if you lose it.
Add your Form 1A.
Upload the signed copy to your application. The Gujarat page has an Upload Document menu for documents and scanned images.
Pay and follow up.
Use Fee Payments, then check progress under Application Status.
Your RTO may still call you in, for example for a driving test after a lapse of more than a year. Sarathi's screens are updated now and then, so go by the labels shown to you.
Surat licensing offices
Section 15 of the Motor Vehicles Act provides that any licensing authority may renew a driving licence.
Surat's offices and the areas they cover can change, so this page does not list them. If you have to attend, Sarathi's Appointments menu for Gujarat offers DL Test Slot Booking and DL services Slot Booking. Confirm the right office and its address with the Gujarat transport department before you go.
Gujarat renewal fees and deadlines
Central fees under Rule 32 of the Central Motor Vehicles Rules apply to licence renewal. Gujarat may show its own total or add service charges, so look at the amount on Sarathi before you pay.

Yes, once you are 40 on a private licence, and at every renewal of a transport licence. Section 15 of the Motor Vehicles Act requires it, and Rule 5 of the Central Motor Vehicles Rules prescribes the certificate. A Surat driver under 40 with a private licence does not need it, unless applying for an adapted vehicle licence due to a physical disability.

How long is a Form 1A certificate valid in Surat?
For six months after it is issued. Before 1 April 2021 a Form 1A stayed valid for a year, until G.S.R. 240(E) amended Rule 5(3) of the Central Motor Vehicles Rules. Check the issue date on your Form 1A before you upload it to your Surat renewal.
What questions are on Form 1A?
Form 1A asks whether you have any defect of vision and whether glasses correct it, whether you can read a motor car number plate at 25 metres in good daylight, and about deafness, night blindness and limb defects. Its certificate also covers colour vision tested on an Ishihara chart, and an optional field records your blood group.
What is the Gujarat driving licence renewal fee?
Rule 32 of the Central Motor Vehicles Rules sets ₹200 for renewal, with another ₹200 for a smart card. Once more than a year has passed since expiry, it is ₹300 plus ₹1,000 for every year or part of a year, and a driving test is ₹300 per class. Gujarat may add its own charges, so check Sarathi.

Yes, within limits. Section 15 lets you renew without a driving test if you apply within one year of expiry. After that, the licensing authority shall refuse renewal unless you pass the test of competence to drive, and the fee rises. Since 2019 the 30 day period after expiry no longer exists.
Can I get Form 1A for a new licence or a learner's licence?
