They do from age 40 when renewing a private licence, and at every renewal of a transport licence. If you need Form 1A, download the one generated for that application and upload it once a doctor has attested it through our service. Then pay the fee and track the application. The RTO may still ask you to attend.

Parivahan, the Ministry of Road Transport and Highways portal, publishes the blank Form 1A as a PDF in English. It is a form under the Central Motor Vehicles Rules. For your renewal, though, download the Form 1A generated for your application number, since the rules require electronic issue on the portal.

A registered medical practitioner. Form 1A states that it is to be filled in by a registered medical practitioner appointed for the purpose by the State Government or person authorised in this behalf by the State Government. Where your RTO has asked for a certificate from a particular doctor, follow that instruction.

Yes. The certificate wording on the official form says colour vision was tested on a standard Ishihara chart and that the applicant has not been found suffering from severe or total colour blindness. The form also asks about defects of vision, reading a number plate at 25 metres in good daylight, deafness and night blindness.

Rule 32 of the Central Motor Vehicles Rules puts the renewal fee at ₹200, and a smart card adds ₹200 more. If you apply more than a year after expiry, the fee is ₹300 with ₹1,000 for each year or part of a year, plus ₹300 per class for any driving test. Check Sarathi for Gujarat's final amount.

We cannot promise that, because acceptance depends on the rules of your RTO. Rule 5(4) says Form 1A shall only be issued electronically on the portal, and in Gujarat you upload the signed digital copy to Sarathi. A printout is not accepted offline, and physical verification needs a physically attested Form 1A.
