What Is the Sarathi Eye Certificate?

The Sarathi Eye Certificate is an official document confirming that your visual standards meet the requirements set by the Ministry of Road Transport and Highways (MoRTH). It must be:

  • Signed and stamped by an Ophthalmologist (Eye Specialist): not a general physician
  • Accompanied by the doctor’s or hospital’s official location stamp
  • Submitted together with Form 1A for licence-related processes in Kerala
This requirement is currently exclusive to Kerala. If you are renewing a licence in any other Indian state, you do not need a separate Sarathi Eye Certificate — Form 1A covers the vision assessment.

At the end of the assessment, the Ophthalmologist certifies whether you are fit or unfit to drive a Category I (non-transport) or Category II (transport/commercial) vehicle.

How to Upload on Parivahan Sewa

  • Save your certificate as a PDF, not a photo or screenshot
  • Keep file size under 1 MB
  • Go to the Application Status section on parivahan.gov.in
  • Upload in the designated certificate field,  Sarathi Eye Certificate and Form 1A have separate upload slots
