Who needs Form 1A in Ludhiana
You need Form 1A in Ludhiana if you are 40 or older and renewing a licence for a private car or two wheeler, or if you are renewing a transport licence at any age. Form 1A rules that apply in Punjab
Form 1A is a national form under the Central Motor Vehicles Rules, and in Punjab the renewal application runs on Sarathi. With our service, a doctor attests your digital Form 1A and you upload it to your Punjab application yourself.

Issuing Form 1A is the doctor's decision. Payment of ₹1,349 for the process is non refundable, so there are no refunds, including when the certificate is not issued. Please read our refund and cancellation policy before you order.
"Can the doctor update the address or photo on my form?"
The doctor does not change the address, photo or other details on Form 1A. Check they are right in your Sarathi application before you order.
"I am 38 and renewing a car licence."
You do not need Form 1A for that renewal, unless you are applying for an adapted vehicle licence due to a physical disability. For a private licence, the rule starts at 40.
"Do I fill in Form 1A myself?"
No. You download the form, and a registered medical practitioner appointed for the purpose by the State Government, or a person it authorises, fills it in.
"My licence expired more than a year ago."
Form 1A will not be the only step. Under Section 15 you must also pass the test of competence to drive.
Renewing a Punjab driving licence on Sarathi
In Punjab, you apply to renew a driving licence on Sarathi, the licence portal at sarathi.parivahan.gov.in.
Open Sarathi and pick Punjab from the list of states.
Choose the driving licence renewal service and submit the application.
Upload the documents, photo and signature that the application asks for.
Copy the application number, and download the Form 1A generated for your application.
Upload the signed digital Form 1A once a doctor has attested it.
Pay the fee when the application asks for it.
The Government of India's services portal lists Punjab's renewal as fully online. After fee payment, it names document scrutiny, biometrics, approval and printing as the remaining steps. If your office asks you to come in, for biometrics or a driving test, follow its instructions.
For old licences, a June 2025 press report quoted Punjab's Transport Minister as saying that applicants could get old driving licences digitised online and then renew them. If yours is an old licence, ask your licensing office whether it needs this step before you apply.

Licensing offices for Ludhiana city and district
Outside Ludhiana city, the Sub Divisional Magistrate (SDM) for your area is the licensing authority for private licences. Ludhiana city is the district headquarters, where that SDM role does not apply.
The Punjab Transport Department's about us page names SDMs, except at district headquarters, as licensing authorities for non transport vehicles at tehsil level. The same page says unit offices headed by Secretaries of Regional Transport Authorities or Motor Vehicles Inspectors handle registration, driving licence and tax work.
A December 2023 press report on the department's decision adds that Regional Transport Officers handle transport vehicle licences across their districts, and private licences only at district headquarters.

The district website lists seven tehsils: Ludhiana East, Ludhiana West, Jagraon, Samrala, Khanna, Payal and Raikot. The department's contact page lists Regional Transport Officers for 23 districts, Ludhiana among them, and the Punjab government's Department of Transport page lists RTA Ludhiana among the posts under the State Transport Commissioner.
Since 2019, Section 9(1) of the Motor Vehicles Act has allowed you to apply for a driving licence to any licensing authority in the state. Confirm the right office for your address before a visit, more so if you live near a tehsil boundary.
Renewal fees and deadlines for a Punjab licence
The central fee to renew a driving licence is ₹200, plus ₹200 for the smart card, under Rule 32 of the Central Motor Vehicles Rules. You can apply from one year before expiry until one year after it, and later than that you must also pass a driving test.

The fees page on the Punjab Transport Department website lists ₹50 for renewal and ₹50 for the test of competence, and gives no date. As these figures differ from the central fees, check the amount due at the time you pay.

Form 1, Form 1A and the official PDF
Form 1A is the medical certificate, while Form 1 was the self declaration of physical fitness. G.S.R. 240(E) omitted Form 1 with effect from 1 April 2021, and the self declaration has since been part of Form 2.
The blank Form 1A is on the Parivahan website: download the official Form 1A PDF. It shows what the form asks. For your renewal, use the copy generated for your application on Sarathi, since the certificate has to be issued electronically on the portal.
What the official form contains
About you.
Name; son, wife or daughter of; permanent address; date of birth; two identification marks; and a photo. A note on the form asks the doctor to sign so that part of the signature is on the photo and part on the certificate.
Vision.
Item 3(a) asks about any defect of vision and whether glasses correct it. Item 3(c) asks whether you can read a motor car number plate at 25 metres in good daylight. The form has no 3(b).
Hearing, night vision and limbs.
Item 3(d) covers deafness that stops you hearing ordinary sound signals, 3(e) covers night blindness, and 3(f) covers any defect, deformity or loss of a limb that affects driving.

The printed text mentions a personal examination, with special attention to distant vision, hearing, and the arms, legs, hands and joints. It also mentions colour vision tested on a standard Ishihara chart, with no severe or total colour blindness.

Fit or not fit to drive a vehicle other than an adapted vehicle, with reasons if not fit, then the doctor's signature, name, designation, registration number, seal and date, and your signature or thumb impression.
