Who needs Form 1A for a Bangalore licence renewal
Whether you need Form 1A depends on your licence type and your age at renewal. A private car or two wheeler licence needs it from age 40, and a transport licence needs it at every renewal.
- Private (non transport) licence renewed at 40 or older: a new Form 1A each time.
- Transport licence for a taxi, goods vehicle or bus: Form 1A at each renewal, at any age.
- Learner's licence or licence for a transport vehicle, or a transport class added to your licence: Form 1A at any age.
- New driving licence applied for at 40 or older: Form 1A is needed.
- International Driving Permit (IDP) application: Form 1A is needed.
- Private licence renewed before you turn 40: no Form 1A is needed, unless you are applying for an adapted vehicle licence due to a physical disability.
Our service covers Form 1A for licence renewals and International Driving Permit applications. It does not cover new licences or learner's licences. On the Karnataka Sarathi page, the permit has its own option, Apply for International Driving Permit (IDP).
How long the renewed licence lasts
Your age on the renewal date sets the new validity, under Section 14 of the Motor Vehicles Act as amended in 2019.
| Licence type and age at renewal | New licence valid for |
|---|---|
| Private, below 30 | Until age 40 |
| Private, 30 or more but below 50 | 10 years |
| Private, 50 or more but below 55 | Until age 60 |
| Private, 55 or more | 5 years |
| Transport | 5 years |
| Hazardous goods | 3 years |
A driver who renews at 41, for example, gets ten more years and meets Form 1A again at the renewal after that.

What applies to Form 1A in Karnataka
Form 1A is a national form under the Central Motor Vehicles Rules, and it applies to Karnataka licences as well. Since 1 April 2021 it has to be issued electronically on the portal and submitted electronically.
- Who fills it in: the form says it is to be filled in by a registered medical practitioner appointed for the purpose by the State Government, or a person the State Government authorises.
- How it reaches Sarathi with us: for Karnataka, you upload the signed digital Form 1A to your Sarathi application yourself. In Tamil Nadu, Maharashtra and Odisha, only doctors empanelled with the transport department can issue Form 1A, and they approve it on the portal. In Karnataka, the doctor does not approve it on the portal for you.
- Your mobile number: the Karnataka Transport Department tells applicants and the public using its services that they must update their own mobile number on parivahan.gov.in, and the Karnataka Sarathi page lists a Mobile Number Update option. Downloading Form 1A from your profile also needs an OTP.
- Office instructions: acceptance depends on your RTO's rules. If your RTO asks for a certificate from a particular doctor, follow that.
Form 1 or Form 1A: what a Karnataka renewal needs
A Karnataka renewal no longer uses a separate Form 1, and it needs Form 1A only if you are 40 or older or hold a transport licence. Form 1, the old self declaration of physical fitness, was omitted by notification G.S.R. 240(E) with effect from 1 April 2021.
The self declaration still exists. It now sits inside the application form, Form 2. Form 1A is a separate document: the certificate a registered medical practitioner completes, valid for six months from issue.

Doubts to clear before you pay
Before you pay, confirm that your RTO accepts an online Form 1A, and read the points below on refusals, corrections and who can complete the form.
Acceptance.
Your RTO's rules decide it. If your Bengaluru RTO has told you to get the certificate from a particular doctor, check with the office before booking.
A refusal.
Issuing Form 1A is the doctor's decision. Payment of ₹1,349 for the process is non refundable, so there are no refunds, including when the certificate is not issued. Please read our refund and cancellation policy before you order.
Corrections.
The doctor will not change the address, photo or any other detail on Form 1A. Check those details in your Sarathi application before you download it.
Filling it yourself.
You can download Form 1A, but only a registered medical practitioner appointed or authorised by the State Government can complete it.
A substitute for a clinic visit.
The online test is not a replacement for a doctor's visit.
Renewing your licence on Sarathi in Karnataka
You can apply to renew a Karnataka licence online on the Karnataka page of Sarathi, which is headed Transport Department, Government of Karnataka. It has an Apply for DL Renewal option, and the department's own website links to its contactless services.
Open Sarathi, select Karnataka and choose Apply for DL Renewal.
Enter your licence details and complete the application.
Save your application number and download the Form 1A generated for it.
Have a doctor complete the Form 1A, then upload the signed copy with your documents. The Karnataka page also lists an Upload Document option.
Pay through Fee Payments and follow progress under Application Status.
A central notification, S.O. 4353(E) of 16 September 2022, allows renewal online where no driving test is needed. If you verify with Aadhaar, which is voluntary, you do not have to visit the RTO. Without Aadhaar you can still apply online, but you must go to the RTO in person to establish your identity.
If you do need to visit, the Appointments menu on the Karnataka page lists DL services Slot Booking. Menu names and the order of screens on Sarathi can change, so go by what you see.
Do you need to visit a Bengaluru RTO?
You are not required to visit a Bengaluru RTO if you verify with Aadhaar and your renewal needs no driving test. You will have to go if you apply without Aadhaar, to establish your identity, or if the RTO calls you in, for example for physical verification.
Before any visit, check your RTO's details with the Karnataka Transport Department. Under Section 9(1) of the Motor Vehicles Act, as amended in 2019, a driving licence application can go to any licensing authority in the State.
If your application stalls, the Karnataka Sarathi page has a File Your Grievance option, and the Transport Department's website gives 1902 as the number for grievances about Karnataka Government services.
Karnataka DL renewal fees and deadlines
The central fee for renewing a driving licence is ₹200, plus ₹200 for the smart card. States may show their own totals or service charges, so pay the amount Sarathi displays for Karnataka.
Fees under Rule 32 of the Central Motor Vehicles Rules
| What you apply for | Fee |
|---|---|
| Renewal, before expiry or up to one year after | ₹200 |
| Smart card licence | ₹200 |
| Renewal more than one year after expiry | ₹300, with ₹1,000 added for every year or part of a year |
| Test of competence to drive, if required | ₹300 for each class of vehicle |
Deadlines under Section 15 of the Motor Vehicles Act
- Earliest application: one year before your licence expires.
- After the expiry date: the 30 day period during which an expired licence stayed valid was removed by the 2019 amendment.
- Up to one year after expiry: renewal without a driving test.
- More than one year after expiry: the licensing authority shall refuse renewal unless you pass the test of competence to drive.
- Your Form 1A: valid for six months from the date of issue, so use it within that time.
Mistakes that can delay a Form 1A
A Form 1A is not valid after six months, and a printout of the digital copy is not accepted offline. Check this list before you upload.
- A certificate more than six months old when you submit it. Book a new one.
- Wrong details on the form. The doctor will not change the address, photo or any other detail, so check them before you download Form 1A.
- An application number copied wrongly. Copy it from your Sarathi application.
- A printout handed in at the counter. Our digital copy works only as an online upload.
- No OTP on your phone. Update your own mobile number on Parivahan before you start, as the Karnataka Transport Department asks.
- A renewal filed more than a year after expiry. You will face a driving test too.
Form 1A format for Karnataka: official PDF
The official Form 1A is a form under the Central Motor Vehicles Rules, and the Ministry of Road Transport and Highways publishes the blank PDF on Parivahan. You can download the official Form 1A PDF to see what it contains.
For a Karnataka renewal, the copy that counts is the Form 1A generated for your own Sarathi application, because Rule 5(4) requires electronic issue on the portal. The blank PDF is useful to see what the doctor fills in.
What Form 1A asks
The form covers eyesight, hearing, night vision and limbs, and ends with a conclusion of fit or not fit to drive.
| Section of Form 1A | What it covers |
|---|---|
| Your particulars | Name, relation (son, wife or daughter of), permanent address, date of birth and two identification marks |
| Photograph | The doctor signs over the photo, so part of the signature falls on the photo and part on the certificate |
| 3(a) | Vision defects, and whether spectacles correct them |
| 3(c) | Reading a car's number plate at 25 metres in good daylight |
| 3(d) | Any deafness that stops you hearing the ordinary sound signals on the road |
| 3(e) | Night blindness |
| 3(f) | A limb defect, deformity or loss that would affect your driving |
| Optional | Blood group and Rh factor |
| Doctor's certificate | The form's certificate refers to a personal examination, with attention to distant vision, hearing, and arms, legs, hands and joints |
| Colour vision | Standard Ishihara chart test. The doctor confirms there is no severe or total colour blindness |
| Only for hazardous goods licences | Glare recovery, side vision and reaction time |
| Conclusion | Fit or not fit to drive a vehicle other than an adapted vehicle, reasons if unfit, and the doctor's signature, name, designation, registration number, seal and date |
| Applicant's mark | Your signature or thumb impression |
Under a note on the form, someone who can hear but cannot speak may still be certified for a non transport licence.

Form 1A in Bangalore: questions and answers
Is Form 1A needed for driving licence renewal in Karnataka?
Yes, when you renew a private licence at 40 or older, and each time a transport licence is renewed. The requirement comes from Section 15 of the Motor Vehicles Act and Rule 5 of the Central Motor Vehicles Rules, which apply in Karnataka as everywhere in India. Below 40, a private licence renewal does not need Form 1A, unless you are applying for an adapted vehicle licence due to a physical disability.
Is Form 1 still needed to renew a licence in Karnataka?
No. Form 1, the separate self declaration of physical fitness, was omitted by notification G.S.R. 240(E) from 1 April 2021. That declaration is now part of the application form, Form 2. Form 1A is a different document: the doctor's certificate needed for private renewals from 40, and for transport licences.
Where can I download the Form 1A medical certificate format for Karnataka?
The official blank Form 1A, prescribed under the Central Motor Vehicles Rules, is published by the Ministry of Road Transport and Highways on Parivahan. For an actual Karnataka renewal, download the Form 1A generated for your own application from your profile on the RTO website, because the certificate must be issued electronically on the portal.
Can I renew my driving licence online in Karnataka?
Yes. The Karnataka page on Sarathi has an Apply for DL Renewal option. Under central notification S.O. 4353(E), renewal that needs no driving test can be done online, and with voluntary Aadhaar verification you need not visit the RTO. Without Aadhaar you can still apply online, but you must visit the RTO to prove your identity.
Who can sign Form 1A for a Bengaluru renewal?
A registered medical practitioner. The instruction on Form 1A says it is to be filled in by a registered medical practitioner appointed for the purpose by the State Government, or a person the State Government authorises. If your Bengaluru RTO tells you to get it from a particular doctor, follow that before you book.
How long is a Form 1A valid after the doctor signs it?
Six months from the date the doctor issues it. Notification G.S.R. 240(E) shortened the period from one year to six months, with effect from 1 April 2021, and the rule applies across India, Karnataka included. If six months pass before you submit it with your renewal, you will need a new certificate.
Do Bangalore drivers under 40 need Form 1A?
No, as long as your licence is only for private vehicles such as a car or two wheeler, unless you are applying for an adapted vehicle licence due to a physical disability. Below 40, Rule 18 does not ask for it at renewal. Transport licences differ: Form 1A is needed at each renewal, and for a learner's licence, licence or added class for a transport vehicle, at any age.
Is a medical fitness certificate for a driving licence the same as Form 1A?
Yes, for a driving licence. The medical certificate a doctor signs for a licence is Form 1A. The self declaration of physical fitness is different: you make it yourself inside the application, Form 2. A fitness certificate for a job, college or sports is another document, so check which one you have been asked for.
What is the DL renewal fee in Karnataka?
Under Rule 32 of the Central Motor Vehicles Rules, renewal costs ₹200 and the smart card another ₹200. Renew more than a year after expiry and the fee is ₹300, with ₹1,000 added for every year or part of a year. A driving test costs ₹300 per class. Karnataka may show its own total, so pay what Sarathi displays.
What does Form 1A cost with MedicalCertificate.in in Bangalore?
₹1,349. That covers the online self test and a digital Form 1A sent to you on WhatsApp or email, ready to upload to Sarathi. Payment of ₹1,349 for the process is non refundable, so make sure you need Form 1A first. Allow up to 72 working hours* after you share your application number, the downloaded Form 1A PDF and complete the ear and eye self test. We work Monday to Saturday from 10:30 am to 8:00 pm IST. Your Karnataka renewal fee is separate, and we do not supply printed or physically attested copies.
Will a Bengaluru RTO accept an online Form 1A?
That depends on the RTO's rules, and we cannot promise acceptance. With us, you upload the doctor's signed digital Form 1A to your Sarathi application yourself. If the RTO calls you for physical verification, you will need a physically attested Form 1A, which we do not provide, because a printout of the digital copy is not accepted offline.
Where can I get a Form 1A medical certificate in Bangalore?
Online with us, or from a registered medical practitioner appointed or authorised by the State Government. You take an eye and ear self test on camera, a doctor attests your Form 1A, and the digital copy comes on WhatsApp or email. Allow up to 72 working hours* from when you share your application number, the downloaded Form 1A PDF and complete the ear and eye self test.
